Ингибиторы CDK4/6 стали стандартом терапии для пациенток с ЭР-положительным/HER2-отрицательным метастатическим раком молочной железы (РМЖ), однако в реальной практике профиль безопасности может отличаться от данных регистрационных клинических исследований (РКИ). Цель: оценить переносимость палбоциклиба и рибоциклиба в сочетании с эндокринной терапией в условиях реальной клинической практики.Материалы и методы. Проспективное открытое одноцентровое нерандомизированное исследование; включе- но 815 пациенток, получавших лечение в период 04.2020–10.2023. Группа палбоциклиба: n=365 (125 мг/сут 21/7), группа рибоциклиба: n=450 (600 мг/сут 21/7). Нежелательные явления (НЯ) оценивали по CTCAE v5.0; применялись описательная статистика и стандартные критерии значимости (p&lt;0,05).Результаты. Любые НЯ зарегистрированы у 33,4% пациенток на палбоциклибе и у 24,0% — на рибоциклибе. Профиль токсичности различался: для палбоциклиба преобладали гематологические НЯ — нейтропения у 112/365 (30,7%) пациенток, в том числе 3–4 степени у 37/365 (10,1%); фебрильная нейтропения — 1/365 (0,3%). Для рибоциклиба чаще отмечалась негематологическая токсичность — астения у 34/450 (7,6%) (преимущественно ≤2 степени); повышение АЛТ/АСТ — 16/450 (3,6%); кардиотоксичность (удлинение интервала Q–T) — 2/450 (0,4%). Диарея и сыпь встречались нечасто в обеих группах. Снижение доз из-за токсичности не сопровождалось ухудшением выживаемости без прогрессирования по данным внутригруппового сравнения.Заключение. В реальной клинической практике ингибиторы CDK4/6 демонстрируют благоприятную переносимость и предсказуемые различия в спектре токсичности (гематологическая для палбоциклиба и преимущественно негематологическая для рибоциклиба), что позволяет индивидуализировать выбор препарата и стратегию контроля безопасности.</p></abstract><trans-abstract xml:lang="en"><p>Background. Cyclin-dependent kinase 4/6 (CDK4/6) inhibitors are a standard of care for ER-positive/HER2-negative metastatic breast cancer (MBC), yet real-world safety may differ from randomized controlled trials (RCTs).Objective. To assess the safety of palbociclib and ribociclib combined with endocrine therapy in routine clinical practice.Methods. Prospective, open-label, single-center, non-randomized study including 815 patients treated between April 2020 and October 2023. Palbociclib cohort: n=365 (125 mg QD, 21/7); ribociclib cohort: n=450 (600 mg QD, 21/7). Adverse events (AEs) were graded by CTCAE v5.0; standard statistics with p&lt;0.05.Results. Any-grade AEs occurred in 33.4% (palbociclib) and 24.0% (ribociclib). Toxicity spectra differed: palbociclib was associated mainly with hematologic AEs—neutropenia in 112/365 (30.7%), including grade 3–4 in 37/365 (10.1%); febrile neutropenia in 1/365 (0.3%). Ribociclib showed predominantly non-hematologic AEs — asthenia in 34/450 (7.6%) (mostly grade ≤2); ALT/AST elevations in 16/450 (3.6%); QT interval prolongation in 2/450 (0.4%). Diarrhea and rash were uncommon in both cohorts. Dose reductions implemented for toxicity did not compromise progression-free survival in within-group comparisons.Conclusions.
