Пациент поступил в многопрофильную клинику с выраженными клиническими проявлениями сердечной недостаточности (СН). При поступлении в стационар в анализах крови была выявлена тяжелая панцитопения, повышение кардиоспецифических ферментов. По данным ЭхоКГ — низкая ФВ ЛЖ, снижение глобальной сократимости левого желудочка, снижение локальной сократимости левого желудочка. Состояние было расценено как острый инфаркт миокарда (ОИМ) 2-го типа на фоне тяжелой анемии. Проводилась интенсивная терапия в условиях отделения реанимации и интенсивной терапии, на фоне которой достигнута выраженная положительная динамика. По ЭхоКГ в динамике отмечалось исчезновение зон нарушения локальной сократимости, увеличение ФВ ЛЖ по Симпсону (с 30% до 42%). При дальнейшем контроле — увеличение ФВ ЛЖ (с 42% до 69%), исчезновение проявлений ХСН. Таким образом, на фоне отмены иматиниба наблюдалось полное обратное развитие проявлений сердечной недостаточности, что свидетельствует об обратимой кардиотоксичности данного препарата.</p><p>Заключение. В клинической практике пациент с впервые выявленной СН требует дообследования на предмет выявления обратимых или необратимых причин, потенциально приводящих к СН. У особой группы пациентов с онкологическими заболеваниями, проходящих специализированное лечение, потенциально может развиваться к развитию кардиотоксичность, в том числе и систолическая дисфункция левого желудочка.</p></abstract><trans-abstract xml:lang="en"><p>This article presents a clinical case of acute decompensation of chronic heart failure in a patient with chronic myeloid leukemia (CML) during targeted therapy with a tyrosine kinase inhibitor — imatinib.</p><p>The patient was admitted to a multidisciplinary clinic with pronounced clinical signs of heart failure (HF). Upon admission to the hospital, blood tests revealed severe pancytopenia and elevated cardio-specific enzymes. According to echocardiography — a low left ventricular ejection fraction (LVEF), decreased global contractility of the left ventricle, and decreased regional contractility of the left ventricle were noted. The condition was interpreted as a type 2 acute myocardial infarction (AMI) due to severe anemia. Intensive therapy was conducted in the intensive care unit, during which a significant positive dynamic was achieved. Follow-up echocardiography showed disappearance of regional wall motion abnormalities and an increase in LVEF according to Simpson (from 30% to 42%). During subsequent follow-up — further increase in LVEF (from 42% to 69%) and resolution of heart failure symptoms were observed.
