Выявление предиктивной BRCA-мутации и назначение таргетной терапии в первых линиях лечения метастатических форм ТНРМЖ может значительно повлиять на продолжительность жизни пациентов. Клинический случай пациентки О. с BRCA-ассоциированным билатеральным метахронным раком молочных желез, демонстрирует, как важно вовремя реализовывать персонализированный подход. В ходе лечения пациентки было важно постоянно сопоставлять первоначальный морфологический профиль опухоли и клиническое течение болезни, чтобы вовремя заподозрить их несоответствие и обратиться к пересмотру операционного материала. Назначение системной лекарственной терапии с учетом генетических и иммуногистохимических особенностей опухоли позволило достигнуть длительные безрецидивные периоды в двух последовательных линиях лечения с сохранением достойного качества жизни пациентки.</p></abstract><trans-abstract xml:lang="en"><p>Triple-negative breast cancer (TNBC), while not the most frequent subtype, has a surprisingly aggressive course and low sensitivity to current antitumour drugs. Detection of predictive BRCA-mutation and administration of targeted therapy in the first lines of treatment of metastatic TNRML can significantly affect the life expectancy of patients. The clinical case of patient O. with BRCA-associated bilateral metachronous metastatic breast cancer demonstrates how important it is to implement a personalised approach in time. During the patient’s treatment, it was important to constantly compare the initial morphological profile of the tumour and the clinical course of the disease in order to suspect a discrepancy and to revise the surgical material.
